✅ Wellness Exams ✅ 

Our patients can stay on top of their healthcare needs with regular wellness exams. Patients have labs drawn on-site and have their needs addressed by one of our providers. The health and well-being of our patients is our priority!

⚠ Chronic Disease Management ⚠ 

Diabetes, hypertension, and other chronic diseases are major issues in the communities we serve. Managing these conditions require regular labs and medication adjustments.

🔎 Cancer Screening 🔎 

We offer screenings for several types of cancer. Through the Well Woman’s Connection program, eligible women receive free cervical and breast cancer screenings. Additionally, we offer lost cost FIT tests that screen for colon cancer.

👶 Prenatal Care 👶 

Through a partnership with nurse midwives at St. Joseph Hospital, we offer care to pregnant people through postpartum care. This includes regular visits with their provider and on-site ultrasounds.

📢 Mental Health Counseling 📢 

We have mental health counselors that provide a listening ear and tools for our patients to process traumas and mental health issues they’ve experienced.

💲 Low Cost Care 💲 

All of our care is offered on a sliding fee scale based on family size and income. We offer services regardless of ability to pay, which is why your support is so important for our clinic to keep providing high-quality care to our patients!
